Initial Assessment

Service Description

At your initial assessment, Dr. Grooms will discuss your needs. A thorough Physical Examination will be performed and any labs and diagnostic tests will be ordered if needed. At that point, Dr. Grooms will create a customized plan specifically based on your needs. Initial Assessment Cash Price is $70. Insured Patients will pay copay at time according to their plan. Individual and Family Plans will be charged according to the information shown on www.drgrooms.org/contact All new patients must first book initial appointment. Initial Assessment price includes adjustments and modalities used during that visit! All payments due at time service is rendered unless prior arrangements have been agreed upon by Dr. Grooms.

Cancellation Policy

To cancel or reschedule, please do so 24 hours in advance. Please call or email us if for any reason you cannot make it to your scheduled appointment. It is our policy that we charge a late cancellation fee of $50 for cancellations within 24 hours of their scheduled appointment.
